The in-game book, Dragon Language: Myth no More, tells the story of how the locals misunderstood dragons. In fact, Paarthurnax and Alduin speak in their mother tongue several times during the main mission, and Alduin even mocks Dragonborn for not speaking the language fluently. The same is true of the Dragon Priests, who can speak both Tamrielic and Dragon Language. Morokei, the Dragon Priestess, even insults a more modern way of speaking, calling it ridiculous language.
We must not forget that the Dragon monks of Skyrim are people chosen by dragons to dominate the rest of the Dragon sect. For this reason, they were seen as superior to humans. Finally, there are draugrs, we should not see these enemies as simple zombies. They are ancient warriors buried centuries before the events of Skyrim, and are among the ancient warriors who spoke in the Dragon Language. This proves that ancient warriors were fluent, or at least knowledgeable in the language of their masters. It also suggests that Dragon Language was the most widely used communication tool in Skyrim in previous periods.
